1. Files Consults, Progress Notes, Assessments and Advanced Directives in Medical Records Charts.
2. Coordinates and follows up with Physicians’ Offices to track Progress of completion of Advanced Directives.
3. Assists Resident Care Coordinator with Transportation Arrangements for Residents to Physician’s Appointments.
4. Copies and distributes Paperwork to Appropriate Departments. 5. Tracks Discharges to Hospital, completes Notifications and prepares Documentation for Signature.
6. Shops for Residents to assure that needs are met.
7. Contacts Families regarding Personal Needs of the Residents.
8. Contacts appropriate Individuals regarding Resident Complaints.
9. Assist in obtaining resources from community social, health and welfare agencies to meet the needs of the resident.
