The Lucas County Board of Developmental Disabilities is seeking social service professionals (social workers, case managers, service coordinators) to develop and implement service programming for individuals with developmental disabilities. Our Service & Supports Department is expanding and we are seeking dynamic team players that can contribute to a positive culture and be a part of our team.
Service and Support Specialists or SSAs, are the primary point of coordination for individuals with an Intellectual or Developmental Disability throughout Lucas County, aged 3 through end of life in community-based services. We assist individuals to live, work, and thrive in their communities. SSAs meet with individuals, families, and service providers to complete assessments, complete linkage and referral to community resources, and authorize Medicaid Waiver services for those who qualify.
SSAs write Individualized Service Plans (ISP) that summarize discovered assessment information, and document how assessed needs will be met. The ISP also identifies outcomes and determines how the team will support the individual to meet their goals. The SSA is responsible for facilitation of team meetings, and monitoring which conducted through home visits and in other community locations.
Caseload sizes are variable but average 30-35 individuals. SSAs are provided all technology to complete their work, including a laptop computer, Mifi hotspot, and internet-based phone system. SSAs have the ability to work hybrid remote up to 16 hours a week, and are offered a Monday-Friday flexible work schedule.
