Job Summary:
Responsible for overseeing and coordinating all aspects of clinical and administrative operations within our hospice care division in accordance with applicable law and regulations. You will play a crucial role in ensuring compassionate and high-quality patient care, regulatory compliance, and operational efficiency. You will lead a team of healthcare professionals dedicated to providing comfort, dignity, and support to patients and their families during this sensitive time.
Essential Functions:
- Develop and implement strategic plans for the hospice program in alignment with organizational goals.
- Ensure the delivery of high-quality care that meets the physical, emotional, and spiritual needs of patients and families.
- Recruit, train, and supervise a multidisciplinary team of healthcare professionals including nurses, social workers, chaplains, and other support staff.
- Foster a collaborative and supportive team culture that encourages professional growth and development.
- Establish and maintain strong, positive relationships with referring physicians, medical practices, and other healthcare stakeholders in the community.
- Provide information and educational materials to referring physicians about our services, specialties, and the latest advancements in healthcare.
- Continuously identify opportunities for collaboration and partnership with referring physicians to increase patient referrals.
- Act as a liaison between referring physicians and facility to address any concerns, issues, or feedback, ensuring a smooth referral process.
- Monitor and analyze market trends, competition, and the healthcare landscape to identify potential growth areas and strategies.
- Maintain accurate records of interactions, referral sources, and outcomes to track progress and performance.
- Work closely with our medical team, marketing department, and other relevant departments to develop and implement referral strategies.
- Gather feedback from referring physicians to continuously improve our services and meet their needs.
- Ensure all interactions and communications with referring physicians adhere to relevant healthcare laws and regulations.
Additional Responsibilities:
- Performs other duties as assigned or requested.
- Conforms to all applicable Agency policies and procedures.
- Participates actively in continuing education and in-services.
- Maintains confidentiality of patient information and business trade practices
- Assumes accountability for reporting incidents and complaints according to Agency policy.

Minimum Position Qualifications:
- Education: Bachelor’s degree in Nursing, Healthcare Administration, Business Administration, or related field
- Experience: 5 years of experience as a Registered Nurse within the Hospice House setting. Minimum of 3 years of progressive experience in hospice care management, with demonstrated leadership and managerial skills
- License / Certification: Current State RN license; additional certifications in hospice and palliative care preferred, current CPR; current driver’s license and proof of auto liability
Environmental Conditions:
Works under a variety of conditions in facilities and offices; ability to work flexible schedule, ability to travel locally; some exposure to unpleasant weather. Moderate noise level; tasks may involve exposure to bloodborne pathogens; moderate stress and emotional demands.
Physical Requirements: Sitting is required. Requires ability to always handle stressful situations in a calm and courteous manner. Requires working under some stressful conditions to meet deadlines and agency needs. Ability to travel.
