What are the responsibilities and job description for the Case Manager/Parent Coach position at Lutheran Community Services Northwest?

The ideal candidate will be mission-driven with a desire to make a difference for people and communities served by LCS Northwest.The Role: The case manager/parent coach will assess the family's needs and provide hands-on parent coaching and education to families who reside in a transitional treatment recovery home in which children are reuniting with families after out of home placement or are at risk of out of home placement. Case manager provides parenting support/education/skills training to parents and provides skills training and/or support to children and adolescents who reside in the home or are in transition. Develops and implements activities which will support attachment and achieve behavioral changes in families where child safety issues are present. This position will also provide lead facilitation for the Parent Education groups. Collaborates with other DHS providers and community agencies, giving oral and written reports. This is a part-time position (21hr week). ESSENTIAL FUNCTIONS:  Provide practical in-home services and support to families to promote, maintain, or restore the safety and well-being of the family membersProvide respite care services in the home or community, including recreational activities and behavioral monitoringCoordinate access and provision of services from multiple agencies, including establishing service linkages, advocating for service needs and obtaining entitlements based on mental or emotional disabilityIdentify family and individual strengths to help resolve child and family safety and attachment concernsAssess parental capacities related to child safety and well-beingProvide supervised visits to children separated from one or both parents to ease the transition from foster care or relative placement to homeEstablish goals based on client living standard choicesTeach and model appropriate play, attachment, and behavior management techniquesCommunicate effectively orally and in writing to answer questions and gather and exchange informationWork with mentally ill, emotionally ill and developmentally delayed individuals and familiesWork with a variety of individuals in difficult circumstances, including angry and hostile clients, disadvantaged individuals, families in provision of services, mentally/emotionally ill individuals, and developmentally delayed clientsWork a flexible schedule to meet families’ needs and be available for crisis consultation, 24 hours a day, seven days a weekMaintain and carry out all scheduled appointmentsKeep accurate records of client services providedIdentify families with drug and alcohol dependence, physically and sexually abused childrenModel and encourage direct, professional communication among staff members and facilitate development of a positive staff support networkDemonstrate awareness and sensitivity in the delivery of client services as it relates to the cultural and socioeconomic characteristics of the populations servedProvide in-home or community services to children, adolescents, and families to support families in promoting, maintaining, or restoring healthy relationships and family cohesionObserve and assess parent child interactions, bonding and attachment, and be able to provide positive feedback and thoughtfully address areas needed for improvement and a plan to do so Coordinate group activities with house manager and other TTRS service related partnersRelate effectively with children, adolescents, parents, staff and the communityDevelop and maintain positive working relationships with TTRS partner organizations and human service providersPrepare curriculum and teach parenting groups as neededProvides variable hours of "hand-on" in-home demonstration, teaching and support to achieve behavioral changes in families where child safety issues are present.   Maintain client confidentialityProvide a range of services including parent education, modeling and mentoring, communication skills, self-care, home management, and links to networks and community resourcesQUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.  Must have well-developed communication skills, both written and verbalMust be neat, organized, and accurateMust be able to work well under pressure and be comfortable in a multicultural settingMust be able to multitask and set prioritiesMust be able to maintain confidentialityHave respectful, open, unbiased, and accepting attitude toward clients and their familiesBilingual (Spanish/English) are encouraged to apply.Coordination tasks are generally performed in a normal office environment.  However, some work is performed in a community-based site (school, church, home, etc.) so travel to and from determined locations will be required.  Transportation of clients participating in the program is also required as needed.EDUCATION and/or EXPERIENCE: Bachelor’s Degree in social work, psychology, counseling, or another related field, or equivalent experience, is required.  Two years of family counseling experience working with high risk families within the last five years, including one year experience working with sexually abused youth, victims/perpetrators of domestic violence, and alcohol/drug addictions is required.  At least two years of experience providing in-home services, such as: budgeting, housekeeping, nutrition, household management, appropriate physical care of children, and parenting skills and techniques are required.
