Position Summary:
  • A Resettlement Services Case Manager provides direct services to refugees, asylees, parolees, special immigrant visa recipients (SIV’s), and other refugee-eligible individuals and families. Case managers may be assigned to a variety of programs including pre/post-arrival, Matching Grant case management, ongoing refugee social services, and extended case management services. The overarching goal is to provide case management services to individuals and families to create both short and long-term goals leading to self-sufficiency and self-reliance. This includes new arrivals, returning clients, secondary migrants, and walk-ins.
Job Duties:
  • Provide quality and timely client-centered services to all program participants, including but not limited to housing, health, legal, and social services (including case management from arrival up to 5 years post-arrival as warranted).
  • Conduct home visits and maintain regular contact to ensure client needs are met and progressing towards self-sufficiency.
  • Comply with funder, program, and organization requirements.
  • Work closely with health, education, and social service providers in the area to provide holistic resettlement and case management services.
  • Provide and/or coordinate interpretation as needed.
  • Complete individualized assessments of clients, addressing both short-term and long-term needs.
  • Document and track client’s progress, case notes, and required data in appropriate systems in a timely manner.
  • Participate in routine peer case file reviews as assigned.
  • Attend and participate in staff meetings for training, team building, and program planning as required.
  • Participate in regular supervision meetings discuss case, strategy, monitor client progress.
  • Utilize internal resources for addressing difficult client situations.
  • Maintain client confidentiality in waiting areas, during phone contacts, while handling records, etc.
  • Work closely with employment and education staff to ensure a client-centered holistic array of care.
  • Demonstrate respect for all people, families, and communities.
  • Ensure adherence to service provision standards and program outcomes.
  • Other duties as assigned.

Education and Experience:
  • Bachelor’s degree in Human Services related field preferred. High school diploma / GED required.
  • Experience providing case management services to individuals and families preferred.
  • Knowledge and experience working with diverse populations preferred.
  • Bilingual in English and another language preferred (desired languages mirror the languages spoken by our client populations, including but not limited to Spanish, Karen, Burmese, Somali, Arabic, Kurdish, etc.).
Physical Requirements:
  • Hybrid work environment (in office, in community, and remote).
  • Company-issued laptop and cell phone.
  • Travel to various office locations to support client services, as needed and as applicable by region.
  • Work a flexible schedule, including evenings and weekends to meet client needs (as directed).
