LSSI is seeking a full-time Outreach Coordinator Monday through Friday, 9:00AM to 5:00PM. This position is both remote/hybrid in office.
This position complies with the Governor’s Executive Order COVID-19 vaccination mandate; which requires full vaccination unless you have a medical or religious exemption. If not vaccinated, weekly COVID-19 testing and results are required.
Duties and Responsibilities:
- Utilize motivational interviewing skills to engage clients in therapeutic and collaborative ways.
- Make initial outreach calls to clients and provide education about the RISE IHPA program and services.
- Complete Transitions of Care Assessments.
- Develop rapport and establish collaborative relationships with hospital staff and Managed Care Organizations.
- Performs other related tasks as needed.
Qualifications:
- High School Diploma with experience in Customer Service. Bachelors in human/social services or psychology is a plus.
- Must have good written and oral communication skills and proficiency in English language.
- Excellent communication, organization, presentation and pc/computer skills (including proficiency with Microsoft Office – Outlook, Word, Excel and PowerPoint) along with other related software.
- Spanish speaking is a plus.
