GENERAL DESCRIPTION
Under general direction of the Human Rights Director, the Human Rights Coordinator is responsible for the coordination of intake services for adults referred to MRI for services and for assisting with human rights investigations. Responsibilities shall include but not be limited to resource development, public information and education, customer service, internal and external communication, the cultivation of potential referrals, and intake services.
ESSENTIAL JOB RESPONSIBILITIES
- Plans, develops, implements and evaluates the pre-established Intake services for adults of MRI by screening and processing referrals and verifying eligibility for service; recommends revision in Policies and Procedures as necessary.
- Responds to telephone referrals from individuals and social service agencies in the community for the purposes of providing an overview of Macon Resources, Inc. (MRI) services and gathering initial information for basic screening.
- After the initial screening process, links adult individuals found to be ineligible for MRI services to other community services, when possible. Develop collaborative relationship with community resources. Acts as liaison between person served/guardian and service coordination agencies.
- Develop working knowledge of MRI services and community resources; learn and stay current on funding sources, requirements, and the intake process with state entities.
- Conduct tours for perspective families, guests, and students; provide introductions to key contacts at MRI.
- Attend IEP meetings and special events as requested/invited at local schools to give families, teachers, and administrators information on available services.
- Completes necessary paperwork per policies and procedures to facilitate admission to the Agency and/or enrollment to services.
- Acts as back-up to Human Rights Director in investigations. Gathers and verifies the required information for human rights investigations. Maintains strict confidentiality to protect all parties involved.
- Serves as liaison with the Council on Quality and Leadership (CQL) in the accreditation process and follow up; accessing the portal; monitoring of the Basic Assurances Plan; monitoring, assigning, and participating in Personal Outcome Measures.
- As a member of the Utilization Review Committee, assists in the planning and coordination of the Utilization Review Committee (URC) meetings; coordinates quarterly utilization/quality reviews;completes quarterly reports; assures meaningful outcome measures; analyzes data and makes recommendations for improvement to service delivery to the QA Committee and the Chief Executive Officer (CEO) for consideration by submitting quality assurance/enhancement summaries and reports on follow up activities.
- Attends Human Rights Committee; records minutes of meeting and submits to supervisor for approval.
- Conducts site visits at MRI residential and training sites; serves as reference and support with staff and persons served; posts rights and reporting information.
- Develops and implements secure storage and maintenance of intake and human rights case files.
- Maintains an ongoing intake log, rights restriction log, OIG case log, and internal complaint log (for each fiscal year).
- Assumes back-up responsibility for Critical Incident (CIRAS) reporting. Maintains Behavior Intervention/Accident/Incident logs.
- Compiles and summarizes data from human rights allegations, abuse/neglect allegations, Accident/Incident Reports, Behavioral Incident Reports and reports findings to supervisor.
- Takes, distributes and maintains meeting minutes for Clinical Review Committee.
- Communicates, retains, and releases information (both written and verbal) on a need-to-know basis and in accordance with the Mental Health and Developmental Disabilities Confidentiality Act and Macon Resources, Inc. Policies and Procedures.
JOB QUALIFICATIONS
- Bachelor’s degree preferred, and at least three years of previous experience in the field of developmental disabilities.
- Must be able to counsel, advise, teach, and resolve conflicts, and support staff, persons served and families.
- Must be able to communicate effectively and diplomatically in both oral and written form and be able to meet and work with the public.
- Must be able to maintain information related to persons served as confidential and in accordance with the Mental Health and Developmental Disabilities Confidentiality Act and the Policies and Procedures of Macon Resources, Inc.
- Must be able to adapt to frequent change and performs adequately when confronted with critical or unexpected issues; make sound clinical judgments based on sound reasoning in daily and emergency situations.
- Must be able to summarize, synthesize and analyze data/information.
- Must have reliable transportation to conduct agency business and/or to transport individuals served; must be 21 years of age; must maintain a valid Illinois Driver's License and a good Motor Vehicle Record; and must maintain personal auto insurance with minimum coverage of Bodily Injury-Liability per Person of $100,000 and Liability per Accident of $300,000, Property Liability of $100,000, Medical Payments of $5,000, Uninsured/ Underinsured Person of $100,000 and Accident of $300,000.
- Must be able to maintain payroll, financial, personnel, and information related to persons served as confidential and in accordance with the Mental Health and Developmental Disabilities Confidentiality Act and the Policies and Procedures of Macon Resources, Inc.
- Must be able to adhere to the Safety Policies and Procedures of Macon Resources, Inc.
- Must be able to complete MRI training requirements.
- Must be able to perform the following:
- Sit and/or stand for prolonged periods of time.
- Bend, twist, stoop or kneel.
- Vision correctable to 20/20 or sufficient to complete job responsibilities.
- Exhibit manual dexterity.
- Adequate hearing.
