What are the responsibilities and job description for the On-Call RN position at Madrone Hospice, Inc?
Definition:
Under the general direction of the Director of Patient Services, the on-call nurse provides telephone, triage and in-person support to hospice patients and their families to ensure quality compassionate care 24/7. Individual shall customarily and regularly exercise discretion and independent judgment.
Qualifications:
Current Registered Nurse license in the State of California
At least one year of nursing experience, preferably in home health and acute care.
Demonstrate ability to assess and respond to the needs of patients and families in varied settings.
Current California driver’s license and proof of insurance, along with a dependable vehicle
Responsibilities:
*In the absence of administrative and management staff, make management decisions as needed based on regulatory requirements and agency policy and procedures.
*In the absence of management staff, supervise and/or provide direction to Hospice House staff.
*Be readily available by telephone and/or cell telephone to respond to patient/family calls as received by Hospice House staff.
*Identify patient/family needs and provide supportive services, referrals or recommendations in accordance with the plan of care and Madrone Hospice policies and procedures.
*Make routine home or hospice house visits as needed to provide patient/family support.
*If a patient is transferred to ER, meet them at the hospital to coordinate and authorize services as appropriate to their plan of care and Madrone Hospice policies and procedures.
*Offer to make visits when death is imminent to provide support, or immediately following to facilitate removal and provide family support.
*Make regularly scheduled visits, as needed, for house patients while on-call.
*Respond to calls in a timely manner.
Functions:
Communicate with the Director of Patient Services, case managers and other staff, as needed, to maintain data on each patient/family currently under hospice care. Maintain a current med sheet for each patient. In-home charts will provide support for home visits.
On-call position is from 4:30pm until 8:00 am the following business day when RN/Case Managers are on duty. Case managers are expected to share the responsibility for weeknight and holiday call as needed.

Pay: $36.66 - $45.64 per hour
