This is a full-time position based in Valdosta, GA and they are looking to interview and fill this position IMMEDIATELY!Base salary range of $100,000 - $120,000.The Home Health Occupational Therapist provides in-home occupational therapy services in accordance with state practice acts, organizational standards, and the Service Agreement. This role focuses on evaluating patient function, developing individualized plans of care, and improving independence, safety, and quality of life in the home setting.Key ResponsibilitiesEvaluate patient functional abilities, home environment, and equipment needs; contribute to the overall plan of careDevelop, implement, and adjust treatment plans and functional goals in collaboration with physicians and care team membersProvide therapeutic interventions, including ADL retraining, adaptive equipment instruction, splinting, and prosthetic/orthotic training as appropriateEducate patients, families, and caregivers on treatment plans and safety strategiesMaintain accurate clinical documentation and communicate changes in patient conditionCoordinate care with interdisciplinary team members and participate in case conferences and in-servicesIdentify needs for additional home health services and make appropriate referralsSupervise Certified Occupational Therapy Assistants and home health aides per policy and state regulationsParticipate in discharge planning and ensure continuity of careRequirementsQualificationsGraduate of an accredited Occupational Therapy programRegistered Occupational Therapist (OTR) with current state licensureMinimum of 2 years of clinical experience with at least one year of home health experience (preferred).Strong communication, documentation, and organizational skillsCurrent CPR certificationValid driver’s license, reliable transportation, and required auto insuranceBenefitsFull benefits!
