TITLE: Social Services Director
REPORTS TO: Administrator
Non-Exempt
PURPOSE OF POSITION:
The primary purpose of the position is to provide interventions that meet the psychosocial needs of residents and families in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the Administrator.
ROLE and RESPONSIBILITIES:
- Ensure completion of DMA-13 for all new admits. Obtain approval for admission from GHP.
- Complete all admission paperwork with all new residents or their family members. Prepare face sheet for the nursing staff on the date of admission.
- Conduct initial interviews with resident/family to obtain social services data within 24 hours of admission.
- Provide residents and/or family members with information regarding application for Medicare and Medicaid.
- Assist resident and/or family member to complete Medicaid application on admission.
- Visit monthly with Case Managers at all referring hospitals.
- Make routine follow-up calls to DFACS to ensure the Medicaid application is proceeding and approval is received timely.
- Ensure completion of DMA-6 for all payer changes.
- Make necessary referrals to PASSR.
- Document timely in medical chart reflecting the resident responses and effectiveness of Social Services intervention.
- Perform clerical duties/administrative tasks timely.
KEY CHARACTERISTICS:
- Highly focused and strong attention to detail.
- Ability to work with frequent interruptions.
- Excellent communication skills written in oral.
EDUCATION/EXPERIENCE/QUALIFICATIONS:
- Must possess a current, unencumbered, active license to practice as a Social Worker in this state.
- Must take the GHCA 36-hour Social Services Director training course.
- Must have at least one year of supervised social work experience in a health care setting working directly with individuals.
