Job Description: Patient Care Specialist
Job Brief:
A Patient Care Specialist is responsible for a variety of tasks including but not limited to: bathing/hygiene assistance, light housekeeping, meal preparation, and transport clients to appointments, errands and/or leisure activities. This position requires a high school diploma or GED, a valid Driver's License, vehicle, proof of auto insurance and a cell phone.
Responsibilities: • Serve as a companion to the client
• Provide medication and appointment reminders
• Perform light housekeeping, household organization and laundry assistance
• Develop meal plan with client and/or primary contact
• Grocery shop and prepare meals
• Provide transportation and accompany client to appointments and on errands
• Assist with bathing, dressing and personal hygiene needs
• Support engagement in recreation and leisure activities
• Monitor and document the client's physical and mental state
• Provide respite care as needed
• Maintains work operations by following policies and procedures
• Document task protocol and complete forms, reports, logs and records
• Conform to HIPP A laws and maintain patient information confidentiality
• Adhere to professional standards, policies and procedures
• Participate in required ongoing training and development programs
Qualifications
• Clear background check and preemployment process
• Valid Wisconsin driver's license with clean driving record
• Reliable vehicle
• Proof of vehicle insurance coverage
• Proficient with the English language: written and verbal
• Minimum 1 years of experience in care giving is field preferred
• Must possess excellent interpersonal and communication skills
• Proven success in working in a team environment
Salary : $24,600 - $31,200
