Field Nurse, Home Care

Licensed Home Care Agency looking for full time RNs to provide superior healthcare services to patients in their homes. Responsibilities

• This role is crucial for ensuring our patients receive the highest quality care possible.

• The ideal candidate will possess a strong clinical background and demonstrate excellent patient assessment skills.

• The RN would be working under the direction of the DPS (Director of Patient Services) to evaluate clients in their homes.

• Conduct comprehensive assessments and aide supervisions in patients' homes.

• Perform thorough head-to-toe patient assessments, including medications and diagnoses, and document findings accurately.

• Supervise and support home health aides/personal care aides in all duties.

• Develop and implement personalized Plans of Care (POC) for aides to follow and provide orientation to the POC. These visits are to assess the client’s homecare needs, create or adjust a Plan of care, and oversee that the caregiver is implementing the plan of care appropriately.

• Instructs and advise patients and/or families as well as the home health aides in their care.

• In addition, there is an occasional need to conduct visits in Orange/Westchester County when needed.

Qualifications

• Current and valid NYS RN License

• Current and Valid Driver's License

• Ability to travel to patient cases within Rockland County

• Must carry Medical Professional Liability coverage

• Experience with Medicaid, Medicare, and Managed Care.

• Experience in home and community-based or long-term care services.

• Proficiency in using the UAS-NY application.

• Experience working with vulnerable populations and a passion for patient care.

• Excellent written communication and documentation skills.
