Job Opportunity: Case Manager - Acute Care Facility (San Antonio, TX Area)
Salary Range: $75k - $85k
44 Licensed Beds, Acute Care Facility

Are you an experienced Case Manager with a passion for improving patient care and outcomes? We're looking for a dedicated Case Manager to join our team in a 44-bed acute care hospital in the San Antonio area. In this autonomous role, you'll work closely with patients, families, and interdisciplinary teams to manage complex cases, ensuring quality care and cost-efficient outcomes.

Position Overview:
As a Case Manager, you will operate under the general supervision of the CFO to advocate for patients, coordinate and negotiate resources, and facilitate optimal care for individuals in crisis. You’ll play a critical role in assessing and managing complex cases, guiding patients and families through health, emotional, and financial challenges.

Key Responsibilities:

  • Interview senior patients in crisis, assess needs, and develop tailored treatment and case management plans.
  • Provide counseling and support to patients, assisting them in problem-solving and utilizing resources effectively.
  • Support family members through individual and group counseling, including caregiver support and grievance groups.
  • Serve as the liaison between patients and other healthcare agencies, ensuring continuity of care.
  • Help patients adapt to changes in their condition, promoting independence and self-esteem.
  • Consult with clinical staff on challenging cases to ensure comprehensive care planning.
  • Maintain accurate case records, reports, and follow-up documentation.
  • Ensure healthcare services are of high quality, cost-effective, and in alignment with patient needs.
  • Apply review criteria to assess medical necessity for admissions and continued stays.
  • Participate in quality improvement initiatives related to patient care management.
  • Uphold all hospital policies and procedures.

Qualifications:

  • Education: Certificate from a five-year college or university program.
  • Experience: At least 1 year of Case Management experience (preferably 3 years).
  • Licensure/Certifications:
  • Healthcare professional with a degree in health science (strongly preferred).
  • BSN or MSN preferred; RN strongly preferred.
  • Skills:
  • 3-5 years of acute care hospital experience preferred.
  • Critical care or emergency department experience highly desirable.
  • Proven expertise in negotiation, communication, conflict resolution, and critical thinking.
  • Familiarity with healthcare financing, community resources, and data analysis.
  • Knowledge of utilization management and third-party payer systems.
  • Experience with managed care preferred.
  • Basic computer skills required.
