- Degree: Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.)
- Education: Graduate from an accredited medical school in the United States or Canada with a degree accredited by recognized bodies
- Certification: Board-eligible or board certified as required
- Internship/Residency: Successful completion of an internship and residency program approved by accrediting bodies
- Experience: Clinical competency within the past two years
