Position Summary:
Max Health Network is seeking a dedicated and organized Home Care Supervisor to oversee and manage the delivery of in-home services for elderly and disabled clients under the Illinois Department on Aging (IDOA) programs. Key Responsibilities:
- Client Care & Case Management
- Conduct in-home assessments for new and existing clients
- Develop, implement, and monitor individualized service plans
- Make changes to care plans as client needs evolve
- Ensure client satisfaction and safety through regular check-ins and follow-ups
Staff Supervision & Scheduling
- Assign, supervise, and support homemakers and caregivers
- Maintain weekly and monthly caregiver schedules to ensure coverage
- Conduct supervisory home visits as required by IDOA (e.g., every 60-90 days)
- Evaluate staff performance and provide training or corrective action as needed
Compliance & Documentation
- Maintain accurate and timely documentation in accordance with IDOA and agency standards
- Ensure all care is provided in accordance with IDOA’s Community Care Program (CCP) guidelines
- Track and report incidents, service interruptions, or quality concerns
Communication & Coordination
- Serve as the primary liaison between caregivers, clients, families, and the agency
- Respond promptly to client and caregiver inquiries or emergencies
- Coordinate with referral sources, case managers, and community resources
️ Training & Support
- Provide ongoing training to homemakers on topics such as client safety, infection control, ethics, and IDOA updates
- Conduct orientation and field training for new staff
- Monitor caregiver certifications (TB tests, background checks, CPR if applicable)

Qualifications:
- Associate’s or Bachelor’s degree in healthcare, social work, or related field preferred
- Minimum 2 years of experience in home care or case management (IDOA CCP experience a plus)
- Strong knowledge of Illinois Department on Aging regulations
- Excellent time management, communication, and leadership skills
- Valid driver’s license and reliable transportation for field visits
- Must pass background check and meet all IDOA caregiver supervision requirements
