What You’ll Do:
- Patient Care: Provide skilled nursing care to patients in their homes, following established care plans.
- Administer Medications: Safely administer medications and treatments as prescribed by physicians.
- Monitor Health: Track and document patient progress, reporting changes to the care team.
- Educate & Support: Educate patients and families on healthcare needs, providing emotional support.
What we are looking for:
- Licensed Vocational Nurse (LVN): Active LVN license in California.
- Experience: Minimum of 1 year of nursing experience preferred, home health experience is a plus.
- Compassionate & Reliable: A caring attitude with a commitment to providing the best care possible.
- Strong Communicator: Ability to communicate effectively with patients, families, and the healthcare team.
- Flexible: Willingness to work various shifts, including some weekends and holidays.
