Description

Registered Nurse-Case Manager

Eglin AFB FL

Work Schedule: The Registered Nurse-Case Manager will be on duty for a minimum of

40 hours per week. Routine work hours will be scheduled by departmental leaders to

correspond with the needs of the department. Scheduled shifts are normally Monday

through Friday between the hours of 0630-1730. Schedules may vary to accommodate

demand. The contract employee will be scheduled Monday through Friday 9 hours per

day (to include a one hour, non-paid, lunch break). Hours scheduled for shifts do not

include travel time involved in reaching the medical treatment facility. The contract

employee shall arrive for each scheduled shift in a well-rested condition and shall have

had at least six hours of rest from all other duties. Any schedule changes are to be

approved by the Functional Requirements Evaluator Designee and must be submitted to

the COR in writing for coordination with the company.

Duties: The duties for the HCW are as follows:

 

Participate in all phases of the Case Management Program (CMP) and ensure that the CMP meets established case management (CM) standards of care.

 

Provide nursing expertise about the CM process, including assessment, planning, implementation, coordination, and monitoring. Identify opportunities for CM and identify and integrate local CM processes.

 

Develop and implement local strategies using inpatient, outpatient, onsite and telephonic CM

Develop and implement tools to support case management, such as those used for

patient identification and patient assessment, clinical practice guidelines, algorithms, CM software, and databases for community resources.

 

Integrate CM and utilization management (UM) and integrating nursing case management with social work case management.

 

Maintain liaison with appropriate community agencies and organizations.

 

Accurately collect and document patient care data.

 

Develop treatment plans including preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions to ensure continuity of care toward the goal of optimal wellness.

 

Establish mechanisms to ensure proper implementation of patient treatment plan and follow-up post discharge in ambulatory and community health care settings.

 

Provide appropriate health care instruction to patient and/or caregivers based on identified learning needs.

 

Participates in the orientation, education and training of other staff. Must serve on committees, work groups, and task forces as needed by the facility.

 

Monitors and evaluates the facility’s case management program, peer review program per DoD, AF and local policies and guidelines.

 

Ensures coordination of care delivery processes, to include alternate healthcare settings and the home environment, for the purposes of enhancing the patient's health and wellness, safety, productivity, and quality of life, and for providing the most beneficial, cost-effective health care.

 

Develops, utilizes and maintains a variety of military and community resources to optimize access to services and medical care. Ensures timely and appropriate provision of services.

 

The CM may need to attend AFMS training in order to remain current with DHA policy and program rules. This may require face-to-face attendance at an out-ofstate location (location TBD) for intensive program updates.

Qualifications


Qualifications:

Degree/Education (5.2.4): The Registered Nurse - The Registered Nurse – Case Manager shall have an Associate’s Degree in Nursing. Graduate from a college or university accredited by National League for Nursing Accrediting Commission (NLNAC), or The Commission on Collegiate Nursing Education (CCNE).

 

Certifications in addition to Basic Life Support: The contractor shall hold current Healthcare Provider (HCP) Basic Life Support (BLS) certification using American Heart Association or the American Red Cross guidelines.  Certification granted based solely on computer-based training does not constitute fulfilling this training requirement. Maintain an active, valid, current, and unrestricted license (with no limitations, stipulations or pending adverse actions) to practice nursing as a registered nurse in any US state/jurisdiction.

 

Experience: 4 years of active nursing practice, 1 months of which must be in discharge planning, clinical case management for adults, children, families’ seniors or groups. Must have worked in nursing in the past 12 months.

 

Licensure/Registration: Current, full, active, and unrestricted license to practice as a Registered Nurse as required in the TO from a United States (US) jurisdiction, state or territory.
