McGregor PACE (Program of All-inclusive Care for the Elderly) is a community-based service program that provides in-home healthcare services to the elderly as an alternative to nursing home placement, allowing Seniors to remain at home.

We are seeking a highly motivated and dedicated Clinic LPN to join our team at PACE. As a Clinic LPN , you will be responsible for supporting day-to-day clinic operations and provides direct nursing care to participants under the direction of a registered nurse.

Responsibilities :

• Within the LPN scope of practice, and pharmacology certification parameters, prepares and administers medications to participants, including oral and parenteral delivery, performs wound care to participants as required
• Within Ohio pharmacy law and LPN scope of practice, prepares medications for participant homegoing and use
• Multidisciplinary team support includes completion of initial comprehensive assessments of participants by enrollment date; assists in the coordination and documentation of a 24-hour care delivery for participants with input from all disciplines of the multidisciplinary team; initial and ongoing clinical observation with oversight of RN of participants with a focus on physical, mental or emotional changes that are to be reported to the multidisciplinary team and reflected on the plan of care as needed
• Assists in departmental and participant outcome data collection and analysis; participates in performance improvement initiatives, as relevant
