At McIntosh Trail Community Service Board, we offer individuals experiencing symptoms associated with mental illness, addictive disease and/or developmental disability the hope for optimal functioning and recovery by providing quality behavioral health services and supports.
Summary
Under general supervision, provides persons, families or vulnerable populations with the psychosocial support needed to cope with chronic, acute, or terminal illnesses and physical or developmental disabilities. Provides patient education and counseling, advises caregivers, and makes necessary referrals for other social services.
Responsibilities
Under direct supervision of clinical supervisor, provide counseling/ therapy services to individuals in an outpatient setting. Duties include but are not limited to
· Complete assessments, individual family, and group counseling services
· Completion of documentation, following all written protocols for timelines and structure.
· Track authorizations and complete reauthorizations / reassessments as necessary.
· Provide crisis intervention when necessary and link individuals to appropriate follow up care. Could involve the use of 1013 process.
· Management of caseload: follow up on treatment recommendations, closure of charts for individuals who are no longer receiving services.
· Participate in weekly treatment team meetings.
· Participate in supervision required to obtain full licensure and complete any needed continued education.
· Perform related duties as assigned by supervisor.
· Maintain compliance with all company policies and procedures.
Minimum Qualifications
· Master's degree in a related field from an accredited college or university AND eligibility to be licensed OR Bachelor's degree in a related field from an accredited college or university AND Two years of experience in social service delivery.
