Case Manager/Social Worker

Meadowbrook Rehabilitation Hospital, located in Gardner, KS is a specialty rehabilitation hospital that provides Acute Inpatient Rehab and Brain Injury Rehabilitation. We are looking for a Case Manager to join our team.

SUMMARY: The Case Manager is the resource person in the area of in-patient case management and discharge planning.

DUTIES PERFORMED:

• Conducts psychosocial history to determine pre-injury issues which need to be addressed.
• Establishes and carries out individualized treatment plan upon completion of psychosocial history if applicable.
• Provides counseling to clients, families and/or significant others.
• Develops and implements family and children support groups as needed.
• Schedules, arranges and moderates the program for family education group.
• Participates in team rounds and family conferences as scheduled through case manger.
• Maintains records and prepare reports according to facility procedures.
• Participates in and documents internal and external professional educational activities to enhance personal and professional growth.
• Performs public relations task as directed by supervisor.
• Coordinates client's schedule with physical therapy, occupational therapy, therapeutic recreation, nursing service, speech and language pathology, behavioral specialist and other disciplines as needed.
• Is an integral part of the interdisciplinary team.
• Assists in patient discharge planning.
• Conducts post discharge follow-up as needed.
• Any other tasks as assigned by the immediate supervisor.

SPECIFICATIONS

• Formal Education/Formal Training: Must hold a degree/license in healthcare.
• Job Related Experience: Minimum of two years experience in a hospital/health care setting, preferably in Inpatient Rehab Facility (IRF)
