MCM has an immediate opening for a Care Coordinator position in our Deerfield location. This is a full-time position that offers competitive pay, medical, dental, vision, and paid time off benefits. The applicant we are looking for will complement the practice and encourage solid customer service skills while being responsible for supporting the patient in every scheduling and communication process.
Responsibilities
- Consult with patients to discuss appointment needs.
- Consult and collaborate with other health care providers and specialists to set up patient appointments.
- Check-in on the patient regularly and evaluate and document their progress.
- Assist the care team with developing and accessing health interventions.
- Treat patients with empathy and respect and conduct oneself in a professional manner.
- Comply with organizational guidelines and health care laws and regulations.
- Maintain patient confidentiality and quality care.
Additional Duties
- Communicate and work regularly with the Office Manager and staff
- Perform job responsibilities with strong attention to detail and accuracy
- Move, carry, and lift objects weighing less than 25lbs without assistance
- Abide by all compliance regulations including but not limited to HIPAA and OSHA
Qualifications
- High School Diploma or equivalent required
- Bilingual preferred
- Experience in medical field is highly desirable.
- Medical Terminology required.
- Knowledge of government CMS regulatory requirements is preferred.
- Computer literate: Microsoft Office (Excel, Word, and PowerPoint) required.
- Electronic medical records systems knowledge a plus.
