Duties and Responsibilities:

1. Assists in appropriate and safe techniques in client’s personal hygiene and grooming that includes bathing, bed bath, sponge, tub or shower bath; shampoo in sink, tub or bed; nail and skin care; oral hygiene; toileting and elimination; foot care, dressing and undressing.

 2.   Assists the client in using urinal, bed pan, commode and bathroom.

3.Assists the client with safe transfer techniques and ambulation, getting client in and out of the chair / bed, including wheelchair.

1. Assists client in normal range of motion exercises and proper positioning in bed or on chair.

1. Makes up bed and tidies up the patient’s room.

1. Assists the patient in carrying out simple prescribed activities, relearning activities of daily living and or basic household skills.

1. Performs those housekeeping tasks which will facilitates the patient’s health at home and are necessary to prevent or postpone institutionalization.

1. Maintains clean, safe and healthy environment.

1. Provides adequate nutrition and fluid intake to the patient. Prepares nutritious foods for the patient and encourages patient to eat.

1. Practices good communication skills. Has the ability to read and write and to understand and carry out directions and instructions, record messages and keep simple record.

1. Reports changes in patient’s status and needs to the RN.  Observations, reporting and documentation of a patient’s status and care or services furnished.

1. Takes vital signs accurately, reading and recording temperature, pulse, respiration and blood pressure.

1. Practices basic infection control procedures.

1. Recognizes emergencies and knowledge of emergency procedures.

1. Attends to the basic elements of body functioning and recognizes changes in body functions that must be reported to an Aide’s Supervisor.

1. Attends to the physical, emotional and developmental needs of the client and finds ways to work with the populations served by the agency including, the need for respect for the patient, his/her privacy or property.

1. Maintains confidentiality of patient’s information and follows good work ethics.

1. Attends patients care conferences and in-service meetings.

1. Respects the Rights of the Elderly.

1. Assists with other patient care procedures by selected trainees which are appropriate in the judgment of a professional nurse and for which the trainee has been especially trained; e.g. taking vital signs, application of hot or cold, and changing clean dressings.

1. Checks with the RN for work schedules and reports after completion of work.

1. Assists with medication ordinarily self-administered, by bringing it to the patient with the proper household measure and water if needed for oral medications.

1. Completes at least 12 hours of Aides’ in-service per year.

1. Performs other tasks that the Agency might assign as allowed.
