What You'll Do:
- Conduct initial and ongoing client assessments.
- Develop and revise personalized care plans.
- Supervise Home Health Aides/CNAs and LVNs to ensure compliance and effectiveness.
- Provide direct care, treatments, and education to clients and families.
- Maintain accurate documentation and report changes in client conditions.
Key Responsibilities:
- Participate in quality assessment and performance improvement activities.
- Oversee ongoing clinical training of care staff.
- Secure and coordinate physician orders for necessary services and supplies.
- Assess home safety and recommend improvements.
- Participate in on-call duties as assigned.
Requirements:
- Education: Graduate of an accredited nursing program.
- Experience: One year of professional nursing experience; home health experience preferred.
- Licensing: Current California RN license and CPR certification.
- Skills: Strong communication and time management skills.
- Transportation: Reliable transportation and valid driver’s license.
- Background Check: Must pass a criminal background check & drug screening.
Physical Requirements:
- Ability to lift up to 50 pounds.
- Comfortable with bending, standing, and navigating stairs.
