GENERAL RESPONSIBILITIES
Provides direct counseling and case management services to clients. This includes, but is not limited to, conducting individual planning sessions, assisting in the development of individual treatment plans, individual case management services and facilitating psych-education groups for appropriate level of care and assisting the client in continuing care placement. This position will provide coverage for specified case loads in residential treatment, partial hospitalization, and intensive outpatient levels of care during times of high census and/or during extended leave times of counselor/case managers.
MINIMUM QUALIFICATIONS
- Bachelors’ degree in counseling or related from an approved program with 1 year experience in alcohol and drug treatment preferred, OR high school diploma, 3 years experience working in the alcohol and drug treatment field, and must be actively pursuing LADAC or other related field licensure.
- Ability to communicate orally and in writing.
- Team Player
- Computer literate with Microsoft.
- Use of EMR experience preferred.
KEY DUTIES
1) With the client and other multi-disciplinary team members, evaluates the client, develops an individualized case management assessment and master treatment plan as well as monitors the clients progress, documenting appropriately as they progress in the case management assessment and maintaining treatment plan goals.
2) Conducts Psycho Education groups in assigned level of care
3) Assists the client in planning for aftercare, making the appropriate referrals.
4) Maintains communications between their program and other Mending Hearts programs ( as well as other agencies to which they are making referrals) to ensure continuity of client care.
5) Assists the Clinical Coordinator in ensuring agency compliance with licensure and other regulations imposed by the state, insurance companies, contracts, etc.
6) Establishes and maintains contact as requested by the referral sources of each client assigned, providing appropriate updates for them, as well as communicates to the support team any client barriers to successful program completion.
7) Participates in community activities and trainings as assigned.
8) Performs other duties as requested by Clinical Coordinator
9) Provides transportation services for clients to assist in connecting with community resources to help in the successful completion of program.
10) Will maintain records that reflect outcomes data as well as report data to the Clinical Staff
