Description:
The behavioral Health Case Manager will be responsible for case management of MCC Clients, work as the liaison with Integrated Behavioral Health and the other community agencies, as well as provide direct service to clients as appropriate.
- Work with Behavioral Health Clinical Team to identify clients to be followed with case management
- Coordinate case management activities for MCC clients
- Work with established collaboratives and other MCC task forces and special programs
- Collaborate with specialty mental health service agencies regarding case conferencing and management
1. Case management experience preferred
2. Experience with mental health clients preferred
3. High School Diploma or GED
