Oahu Case Manager

Mental Health Kokua (MHK) is hiring a full-time Case Manager to join our team on Oahu. In this impactful role, you’ll support adults living with mental illness in the Oahu community—helping them access behavioral health care, develop independent living skills, and work toward stability and wellness across the island.

Case Manager Responsibilities:

• Conduct behavioral health assessments and develop person-centered service plans.
• Coordinate treatment, referrals, and entitlements for consumers.
• Provide follow-up support after hospitalizations and during ongoing care.
• Collaborate with medical staff and monitor consumer progress.
• Offer transportation, budgeting guidance, and recovery-based support.
• Maintain timely and accurate clinical documentation.
• Participate in supervision and training to ensure quality services.

Case Manager Qualifications:

• Education: High school diploma or equivalent required; bachelor’s degree preferred—especially in psychology, social work, or a related behavioral health field.
• Experience: Prior experience supporting individuals with mental illness or co-occurring disorders is highly valued.
• Skills: Strong communication, collaboration, and critical thinking skills; able to respond with empathy, flexibility, and cultural sensitivity.
• Licensing & Compliance:
  • Must pass a criminal background check and drug/alcohol screening
  • Maintain current TB clearance (or TB X-ray), CPR, and First Aid certifications (or be willing to obtain)
  • Valid driver’s license, proof of insurance, current vehicle safety inspection, and clean driving abstract
• Physical Requirements: Able to lift up to 25 lbs, travel between program sites, and perform physical tasks such as bending, stair climbing, and standing for extended periods.
