Schedule: 1st shift, Monday-Friday, plus on call rotation
Position Details
The case manager plays a pivotal role as the recovery coordinator for our community-based Dual Diagnosis Treatment Team (DDTT) serving Berks County. The DDTT focuses on a recovery-oriented approach supporting individuals diagnosed with both a mental illness and intellectual disability.

Position Details

Duties and Responsibilities
- Collaborate with other members of the team to the implement recovery-oriented, person-centered treatment and support plans.
- Performing initial intakes, assessments, and timeline summaries.
- Provide intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, and natural support residences.
- Provide individual and group treatment and support according to each individual’s/family’s needs.
- Support data collection by monitoring interventions and providing feedback to the treatment team regarding client progress.

Our Ideal Candidate Will Possess The Following Skills
- Bachelor’s degree in Social Work, Psychology, or another human service-related field
- Active driver’s license with a clean driving record
- One year of experience working behavioral health is preferred
