What are the responsibilities and job description for the Drug and Alcohol Case Manager position at Merakey?
Merakey is currently seeking a Drug and Alcohol Case Manager to join our Behavioral Health Division in Philadelphia County, PA location.
POSITION OVERVIEW
This role is responsible for coordinating and implementing services for consumers as identified in the service plan. This position will serve as primary contact for no more than 30 individuals. You will be providing case-management, service planning and coordination of services with other agency providers to meet the clients' needs along recovery supports. May include on-call responsibilities.
Case Management:
- Primary case manager for assigned consumers. Primary responsibilities include minimal bi-weekly support contacts at the client's home or wherever most appropriate and completion of consumer-driven service and safety plans. If unable to contact the client, attempts will be made to locate them through family, friends, or relatives.
- Assist with linkage to external resources such as, but not limited to: recovery-based supports such as 12-step meetings, medical benefits, PCP, specialty doctors, food banks, transportation, obtaining and maintaining basic living needs and skills related to housing, food, psychiatric and medical care, transportation, socialization, education, and employment. Also, assist consumers in obtaining valid, state identification when applicable.
- Coordinate service delivery with other service providers and monitor the client's progress.
- Monitors record keeping and authorized units remaining, ensure that client charts are up-to-date and include all necessary documentation, including documentation of significant incidents within 24 hours as needed (incident reports) and non-billable activities.
- Ensure that the consumer receives an annual physical and monitor their medical health by requesting medical documentation.
- Secure past records, understand consumer's past and present life situation, and utilize that information along with meeting with the consumer to develop a social history and psychosocial summary, which will be updated yearly for each consumer on primary caseload.
- Maintain accurate summaries of the client's hospitalizations, crisis on-call (if applicable), crisis residential services and incarcerations with dates of service, reason for the services and discharge dates.
- Ensures timeliness, quality, and appropriate signatures on treatment plans every 60 days, and daily progress notes handed in weekly or as required.
- Adheres to all Merakey, County, State, and Federal policies and regulations related to Drug and Alcohol treatment services, ethics and code of conduct.
- Participates in regular clinical supervision, team meetings, and continuing education activities to further develop professional skills and to meet DDAP requirements.
- Participates in Merakey quality improvement/quality assurance activities.
- Performs other duties as assigned as it relates to the position.
- Complete all paperwork in an accurate and timely manner.

Requirements:Bachelor's Degree in a related field (psychology, counseling, human services, or social work)
Actively working toward credential through PCB (i.e. CPS, CRS, CAAC, CADC, CAADC, CCS to be achieved within two to three (2 - 3) years of hire, depending on requirements.
Hires after 7/1/2021 – employees must complete and maintain certification and/or license required by regulations and offered by Merakey within the required timeline. Failure to successfully complete and maintain license and/or certification and associated continuing education requirements will prevent you from being able to fulfill this role.
