Position Summary:
Under the direction of the Program Manager, The Community Health Worker is responsible for conducting outreach and education activities for identified populations. The Community Health Worker is responsible for identifying possible outreach and education locations and effective strategies to reach target population. The Community Health Worker will lead education sessions to both groups and individuals. The Community Health Worker will adhere to established procedures for outreach, education, advocacy and client referral. The focus area for The Community Health Worker will vary according to program goals. The Community Health Worker must become knowledgeable of focus area content, approved messaging and required procedures to advocate for clients. Qualifications
Minimum Qualifications:
- A minimum of one-year experience in community outreach, education advocacy work
- DRV LIC.
- CAR INS
- Bilingual in English and Spanish
