Job Summary: The wound care nurse is responsible for coordinating comprehensive wound assessments upon admission, newly formed ulcers and weekly for residents on bed-rest. Provides education to residents, family and staff regarding prevention of pressure ulcers, therapeutic support services, dressing selection and adjunctive therapy. Collaborates with clinical staff, physicians and other members of the treatment team to provide the resident with the highest quality and most appropriate care. Acts as a resource for the treatment team and clinical staff.Summary of Essential Job Functions: •Perform skin assessment on all newly admitted residents the admission assessment is completed within 24hr of admission•Wound assessments and evaluation and documentation of patient’s clinical condition are updated weekly as established in department guidelines, policies and procedures and regulatory guidelines•Assists with preparation for rounds with wound care doctor on appropriate days to include review of charts, ensures wound care cart is stocked appropriately and that dressing changes are occurring per established schedule•Provide teaching and guidance to patients/residents, family and staff in wound care, customizes instruction based on the specific individual’s level of understanding•Evaluates compliance with established standards and regulatory agencies and takes appropriate action•Participates in Quality Assurance Program and risk management activities, reviews reports and collaborates with administrative support services to provide a clean and secure environment for patients, staff and visitors•Participates in department meetings, in-services and trainings as requested•Updates the DON of Wound Care Status and concerns weekly and as needed•Attend weekly Nutrition at Risk meeting with the Dietary DepartmentMinimum Qualifications: Education: Graduate from an accredited school of Nursing, Experience: •Two years experience in wound care preferred•Long Term or acute care experience preferredLicenses/Certifications: •Current RN License in the Florida State Board of Nursing •BLS Certification required•Certification in wound care a plusAbilities Required: •Good clinical assessment skills•Excellent interpersonal communications skills, and the ability to teach •Knowledge of long term care•Computer literacy required •Ability to think and act calmly and logically in unusual occurrences •Demonstrated ability to receive, transcribe and carry out physician’s orders •Demonstrates a caring attitude towards others •Demonstrates knowledge in Wound Care assessment•Coordinates, follow up plan of care using nursing process•Comfortable in doing Wound Care Treatment and dressing as ordered by the physicianWorking Conditions: Environment Work Conditions: •Normal working condition with adequate lighting and ventilation•Occasional exposure to aggressive patients•Ability to stand or walk for 90% of the shift •Ability to bend and stoop frequently •Ability to do frequent lifting and turning of patients/residents, singularly and with other staff members and equipment Infectious Material Exposure: Routine exposure to blood/body fluids/tissues
