Registered Nurse (RN) Care Manager
Locations: Big Rapids, Reed City, and Stanwood, MI
Schedule: Full-time, Monday–Friday, 8-hour shifts

Position Summary
We are seeking a compassionate and skilled RN Care Manager to provide care management to chronically ill patients. In this role, you will partner with patients, their families, providers, and community resources to promote self-management, facilitate care transitions, and improve overall health outcomes.
Key Responsibilities
- Develop patient-centered care plans and coordinate services across the care team.
- Manage transitions from inpatient to outpatient care to reduce hospital readmissions.
- Educate patients and families on disease management, medications, and lifestyle modifications.
- Monitor progress toward care plan goals and adjust interventions as needed.
- Build strong relationships with community agencies to address social determinants of health.
- Document all patient interactions and care plans in the electronic health record (EHR).
- Support Chronic Care Coordinators and collaborate with providers during team meetings.
- Travel between MPCP locations (Big Rapids, Reed City, Stanwood) as needed.
Qualifications
- Active RN license in Michigan.
- Minimum 1 year of clinical nursing or case management/managed care experience required.
- Experience in a physician office or clinic setting preferred.
- Strong organizational, communication, and problem-solving skills.
- Proficiency in electronic health records and basic computer applications.
- Ability to work both independently and collaboratively with a multidisciplinary team.
- Valid driver’s license and reliable transportation required.
