Personal Choices Coordinator Job Description

The Personal Choices Coordinator is responsible for enrolling clients in the Personal Choices Program, developing their support plans and budgets, assessing the quality of services and the need for continued service, maintaining service records, preparing reports for the Alabama Department of Senior Services (ADSS), conducting home visits, and monitoring clients' services.

Key Responsibilities:

  • Caseload Management: Handle up to 70 clients on the Medicaid Waiver Service Program for the Elderly and Disabled.
  • Client Outreach: Contact potential clients to discuss program requirements and responsibilities, and arrange in-home visits if appropriate.
  • Orientation and Training: Provide clients with orientation and training on the Personal Choices program responsibilities and requirements.
  • Support Plan Development: Assist and guide clients in developing a personal support plan to meet their personal assistance and health care needs.
  • Budget Evaluation and Monitoring: Evaluate and approve clients' budgets to create a reasonable spending plan and allocate dollars effectively. Monitor monthly budget expenditure reports from the Financial Management Service Agency to ensure compliance with the approved support plan and identify spending patterns that may indicate issues.
  • Plan Approval: Forward personal support plans to the Personal Choices Program Manager at ADSS for approval and notify the Financial Management Service Agency upon approval.
  • Quality Assessment: Make monthly calls to new enrollees to assess the quality of self-directed care, provide necessary support, and ensure essential needs are met.
  • Home Visits: Conduct semiannual in-home visits, averaging 15 visits per month, to further assess care quality and client safety.
  • Collaboration: Work with clients’ case managers to ensure their continued health and safety.
  • Record Keeping: Maintain comprehensive records for each client enrolled in the program.
  • Representation: Represent the Area Agency on Aging in presentations, committees, workgroups, and all matters related to the Personal Choices program.
  • Service Delivery Oversight: Oversee and document the service delivery of the MWS Elderly and Disabled Waiver for the caseload.

Additional Duties: Perform other duties as assigned.

Knowledge, Skills, and Abilities (*can be acquired on the job)

  • Ability to effectively interpret policies and procedures established by applicable projects.
  • Comprehend the goals, objectives, and regulations of the program.
  • Ability to analyze program guidelines and make decisions.
  • Knowledge of English grammar, spelling and punctuation.
  • Ability and experience in expressing ideas effectively both orally and in writing.
  • The ability to establish and maintain effective working relationships with staff, sponsors, state agencies, contractors, clients and their families.
  • Good record keeping skills.
  • Demonstrated Microsoft Office expert skill level and ability to quickly and proficiently learn new software.
  • Ability to develop and/or manage multiple projects.

Education / Experience:

1. Bachelor’s Degree in social work, psychology or related field. Preferred experience in social work, especially the geriatric population.

  • Ability to meet the attendance requirements for the position.
  • Ability to “drive” to locationsAbility to occasionally lift and/or move up to 10 pounds.

Job Type: Full-time

Pay: $19.23 per hour

Expected hours: 40 per week
