Community Care Hospice is a dedicated hospice care organization committed to providing compassionate end-of-life care for patients and their families. Serving our rural communities, we believe in delivering quality care that meets the unique needs of each individual.
Employment Type: Part-time| PRN | M-F 8am-5pm
Salary Range: $26/hr (pay is based on qualifications and experience)
Location: McPherson, KS, and surrounding area
We are seeking a compassionate and experienced Hospice Social Worker to provide emotional support, resource coordination, and end-of-life planning for individuals receiving hospice services. This role is vital in helping families navigate the physical, emotional, and financial challenges during this meaningful time.
Key Responsibilities:
- Provide emotional support, crisis intervention, and grief counseling to patients and families
- Assist with advance directives, end-of-life planning, and coordination of community resources
- Collaborate closely with the interdisciplinary team, including nurses, chaplains, and physicians, to ensure comprehensive patient care
- Support patients and families through the bereavement process, including post-death follow-up
Qualifications
Required: LMSW or LBSW in Social Work. Current unrestricted State of Kansas license to practice as a Social Worker. At least two years of social work experience in health care or mental health. Experience working with older adults is preferred. Must possess a valid Kansas Driver’s License, in force automobile liability insurance and maintain a Motor Vehicle Record (MVR) that meets organization’s insurability standard.
