SUMMARY DESCRIPTION OF CLASSIFICATION:
Under the direction of the Middle/High School Coordinator for Student Support Services, the Secondary Transition Coach will be responsible for the transition of students to the community by implementing the person centered planning process in support of students’ secondary transition goals. The Secondary Transition Coach will coordinate transition activities within a team composed of the student, parent/guardian, educator and service providers, to ensure that the activities are appropriately implemented.
KNOWLEDGE, SKILLS, AND ABILITIES:
Thorough knowledge of community resources and vocational services.
Thorough knowledge of statutory obligations related to students aging out from programmatic offerings.
Ability to work collaboratively with agency representatives and community partners as part of the coordinated set of activities required in transition services.
Skilled in working with students towards developing soft and hard skills for post schooling opportunities.
Knowledge of transition law, and transition service requirements.
Ability to effectively communicate with staff, administrators, students, teachers, parents.
EDUCATION, TRAINING, AND EXPERIENCE:
Associate’s Degree Required. Bachelor’s Degree in Education, Psychology, Human Services or related area of study preferred.
Such alternatives as deemed acceptable and appropriate by the Milford School Board.
CERTIFICATE AND LICENSE REQUIREMENTS:
Transition Specialist Certificate or equivalent or preferred.
SCHEDULE:
1. Monday - Friday, 3.25 hours per day, maximum 16.25 hours per week.
