Summary:

This is a professional position providing a wide range of services within the scope of the field of training. Persons in this position are responsible for providing intake services to adults and children experiencing severe and persistent symptoms of mental illness (SPMI) and mild to moderate mental illness. This often includes the coordination of services with families, natural supports, members of the community, psychiatrists, other team members, primary care physicians, and social service and collateral service agencies in order to work toward treatment goals. Assessments and testing g, may be performed based on individual need. Employees in this position function with a great deal of professional independence under the general direction and supervision of the Access Intake Director.

Responsibilities and Duties:

  • Conducts in person comprehensive assessments and evaluations
  • Provides direct services based upon the individual' s assessed needs to help them progress toward their person-centered goals
  • Provides interventions based upon individual need such as individual, family, group, and activity therapy and referrals when applicable
  • Responsible for direct service productivity and documentation standards per program expectations
  • Collaborates with other clinical staff in planning and conducting treatment and activities
  • Participates in case conferences and staff meetings
  • May schedule psychiatric evaluations and medication reviews, as needed
  • Acts as a liaison between the individual/family and their natural supports or providers of services in the community
  • Assists the family to access and utilize appropriate community resources (community inclusion).
  • Participates in in-service planning and programming
  • Participates in the planning, development, and evaluation of agency programs
  • Must meet agency productivity/performance standards
  • Must adhere to agency policies and procedures
  • Adhere to all requirements /standards of the Behavioral Health Telehealth Agreement
  • Other tasks and responsibilities, as assigned

Minimum Qualifications:

  • Master's degree in Social Work, Psychology, Counseling or a related mental health discipline Appropriate licensure from the State of Michigan
  • A minimum of one (1) year experience with the evaluation/treatment and services delivered to adults and/or children diagnosed with mental illness.

Job Type: Full-time

Pay: $42,428.00 - $59,096.00 per year
