Job Summary:
All employees are expected to perform their duties in alignment with the vision and values of the organization. The Case Manager is responsible for a variety of tasks that lead to a high level of customer satisfaction in the most cost-effective manner. Responsible for case management activities(including utilization management and discharge planning activities) on designated units within the hospital.
Qualifications
Required
- Minimum Education: Licensed Vocational Nurse Degree or Associate Degree in Nursing.
- Bachelor’s Degree in Nursing preferred.
- Specialized Training: Case Management, Utilization Management or Discharge Planning preferred.
- CCM or ACM certified preferred.
- Licensure: Licensed Vocational Nurse or Registered Nurse with current and valid
- California License.
- Software: Basic Microsoft computer skills preferred.
