As the Treatment Plan Nurse for the BHU (Behavioral Health Unit) at Mission Community Hospital, you will play a crucial role in providing individualized and holistic treatment plans for patients suffering from mental health and psychiatric disorders. - Conduct comprehensive psychiatric and biopsychosocial assessments of patients upon admission to the BHU
- Develop individualized treatment plans for patients in collaboration with the interdisciplinary treatment team
- Monitor and evaluate patient progress towards treatment goals on a regular basis
- Communicate with patients and their families to educate them on treatment plans and provide support
- Coordinate and facilitate interdisciplinary team meetings to discuss patient progress and make necessary adjustments to treatment plans
- Assist in the delivery of evidence-based therapy and interventions as needed
- Document all patient interactions and progress in electronic medical records
- Collaborate with community resources to ensure seamless discharge planning and continuity of care for patients
- Adhere to all hospital policies, procedures, and standards of care
- Participate in ongoing quality improvement initiatives for the BHU
- Active RN license in the state of California
- Bachelor's or Master's degree in Nursing from an accredited institution
- Minimum of 3 years of experience in a psychiatric or behavioral health setting
- Strong knowledge of mental health disorders, treatments, and medications
- Excellent assessment, critical thinking, and problem-solving skills
- Ability to work effectively as part of an interdisciplinary team
- Strong communication and interpersonal skills
- Experience with electronic medical records
- CPR certification
- Bilingual (English/Spanish) preferred but not required
