As a Licensed Vocational Nurse (LVN) at our Grover Beach branch, you will play a vital role in delivering high-quality home health care to our patients. You will work closely with a multidisciplinary team to ensure that patients receive the best possible care in the comfort of their own homes.
Key Responsibilities:
- Provide direct nursing care to patients in their homes, following established care plans
- Monitor patients' conditions, report changes, and collaborate with the care team to adjust care plans as needed
- Administer medications and treatments as prescribed by physicians
- Educate patients and their families on disease management, treatment options, and health maintenance
- Maintain accurate and timely documentation of patient care and progress
- Current and valid LVN license in the state of California
- Wound care preferred
- Minimum of 1 year of experience in a clinical or home health setting preferred
- Strong clinical skills with the ability to work independently
- Excellent communication and interpersonal skills
- Compassionate and patient-centered approach to care
