We offer competitive pay along with a comprehensive benefits package.Home Health / Hospice Nurse Job DescriptionReceive patient assignments from the Operations Leader or Clinical Resource Team Leadermunicate effectively regarding patient care, business operations, and topics that foster a positive work environment.Assume responsibility for patient and family care, including assessment, planning, implementation, and evaluation.Initiate and maintain communication with attending physicians, hospice staff, and other agencies to ensure optimal patient care and resource utilization.Collaborate regularly with the Operations Leader or designated personnel to review care strategies.Maintain consistent communication with attending physicians about patient and family care.Collect data on physical, psychological, social, and spiritual factors influencing patient and family health, and integrate this information into the care plan.Home Health / Hospice Nurse CertificationsMinimum of two years of nursing experience preferred; one year in home care is highly desirable.Current BLS Certification.Valid Registered Nurse licensure in the state of South Carolina.Valid drivers license and proof of vehicle insurance.If you are passionate about providing outstanding home health care and meet the qualifications listed above, we invite you to apply and become a valued member of our caring team!
