JOB DUTIES
Assesses the social and emotional factors, socio psychological impact of illness, need for care, response to treatment and adjustments to care for patient.
Provides direct social work and counseling services to patients and their families.
Attempts to meet identified needs of patients by utilizing available patient, family and community resources as applicable.
Provides assistance with practical environmental problems and financial assistance needs as necessary.
Assesses continued productivity - manages caseload.
Prepares and maintains accurate patient records including clinical and progress notes and related reports in accordance with Agency policies.
Assists physician and other team members in understanding the significant social and emotional factors impacting the health problems of the patient.
Serves as a consultant to external Agencies or other internal department personnel to aid their understanding of the social and emotional factors affecting the patient.
Keeps manager informed on regular basis of job duties and of changes in status as needed.
Communicates accurately and effectively with external and internal customers.
Acceptance of additional duties as assigned.
Attends workshops, in-services and/or other professional meetings, as preferred to enhance one's knowledge in their field of work.score: N/A

SPECIFIC JOB DEMANDS
Strength: Light Work - Lifting, Carrying, Pushing, Pulling 20 Lbs. occasionally, frequently up to 10 Lbs., or negligible amount constantly. Can include walking and or standing frequently even though weight is negligible. Can include pushing and or pulling of arm and or leg controls.
Reaching: Frequently - Extending hand(s) or arm(s) in any direction.
Handling: Frequently - Seizing, holding, grasping, turning, or otherwise working with hand or hands. Fingers are involved only to the extent that they are an extension of the hand, such as to turn a switch or shift automobile gears.
Fingering: Frequently - Picking, pinching, or otherwise working primarily with fingers rather than with the whole hand or arm as in handling.
Talking: Frequently - Expressing or exchanging ideas by means of the spoken word to impart oral information to clients or to the public and to convey detailed spoken instructions to other workers accurately, loudly, or quickly.
Hearing: Frequently - Perceiving the nature of sounds by ear.
Near Acuity: Frequently - Clarity of vision at 20 inches or less.
Accommodation: Frequently - Adjustment of lens of eye to bring an object into sharp focus. This factor is required when doing near point work at varying distances from the eye.
Color Vision: Frequently - Ability to identify and distinguish colors.
MINIMUM EDUCATION, KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED
Bachelor or Master’s Degree from a school of social work accredited by the Council on Social Work
One-year experience in social work, preferably with a medical background. Prior computerization experience preferred.
Credentialing/Competencies in accordance with current credentialing and/or competency requirements.
Licensed, or access to transportation, driver with current Pennsylvania License and careful driving record.
Auto insurance is required by the State of Pennsylvania and proof of current coverage is required at all times.
BLS required.
WORKING CONDITIONS
Work setting may be either that of an office or in the home involving the ability to bend, stoop, lift, twist and maneuver as necessary to conduct patient evaluations, and to access the patient's home.
Possible exposure to various chemicals relating to job performance would be in the use of equipment and in all clinical/medical procedures which may be performed.
Ability to hear and communicate in the English language (written and oral), as relative with family and Agency staff regarding same.
Position requires traveling both day and night therefore, requires ability to drive or access to transportation providing staff with ability to provide these services.
