Overview: We are seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our skilled nursing facility rehab unit and long-term care unit This position offers the opportunity to provide comprehensive medical care, manage chronic conditions, and collaborate closely with interdisciplinary teams to ensure high-quality patient outcomes in a supportive and dynamic environment.

Responsibilities & Qualifications

  1. Patient Care:
    • Conduct thorough initial assessments, develop care plans, and perform routine follow-up evaluations for patients in the rehab and long-term care units.
    • Manage acute and chronic medical conditions, including but not limited to diabetes, hypertension, respiratory diseases, and neurological disorders.
    • Prescribe appropriate medications and treatments, adjusting them as necessary based on patient response and evolving care needs.
  2. Collaboration:
    • Work closely with attending physicians, nursing staff, physical therapists, and other healthcare professionals to coordinate comprehensive patient care plans.
    • Participate actively in interdisciplinary team meetings to discuss patient progress, treatment plans, and discharge planning.
  3. Documentation and Compliance:
    • Maintain accurate and up-to-date medical records, ensuring compliance with facility policies, state regulations, and billing requirements.
    • Complete documentation in a timely manner, including progress notes, medication orders, and discharge summaries.
  4. Patient Education:
    • Educate patients and their families on disease prevention, treatment options, medication adherence, and lifestyle modifications to promote overall well-being.
    • Provide counseling and support to patients facing complex medical decisions or challenges.
  5. Quality Improvement and Training:
    • Participate in quality improvement initiatives, clinical audits, and performance improvement projects to enhance patient care outcomes and facility practices.
    • Mentor and provide clinical guidance to nursing staff and other healthcare providers as needed.

Qualifications:

  • Current licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) in the state of NY.
  • Certification as an NP or PA from an accredited program.
  • Previous experience in geriatrics, rehabilitation, or long-term care preferred.
  • Strong clinical assessment skills and the ability to make autonomous clinical decisions.
  • Excellent communication skills and the ability to collaborate effectively within a multidisciplinary team.
  • Proficiency in electronic health records (EHR) systems and medical documentation

Home and assisted living patients are centered primarily in Buffalo, NY. Skilled nursing facilities extend across the state of NY.
