Qualifications:
- Graduate of an accredited vocational school of nursing
- Current Minnesota LPN license (or equivalent)
- Previous experience in Long Term Care, Home Health, acute care nursing, or Hospice (preferred)
- Ability to collaborate with others
- Strong communication and human relation skills
- CPR certification
- Written and verbal communication skills
- Valid drivers license (required)
- Computer literacy
Responsibilities:
- Perform patient visits under the supervision of an RN Case Manager
- Provide patient care as delegated by the RN
- Monitor nursing care provided by other personnel
- Assist with data collection and plan of care development
- Utilize iPad for client documentation
- Adhere to clinical nursing practice standards and the Nurse Practice Act
