The Licensed Practical Nurse performs all nursing functions within the scope of practice of a LPN in the clinic setting to facilitate patient flow and care by the provider. Performs a variety of activities characterized by change and short duration in caring for patients including scheduling, triage, working with a variety of Providers, authorizations, and referrals. Will be trained and work in Primary Care Clinic when needed.
• Documents care and assessments,
• Ensures proper documentation of treatment(s) and patient information is ready for each provider
• Provides follow-up patient care by communicating test results to patients and relaying provider plan of care.
• Identifies health education needs of patient; develops and implements specific teaching plan appropriate to age and/or developmental stage.
• Uses the computer and telephone to communicate effectively and accurately document medical and demographic information.
• Provides front desk assistance, such as answering phone, filing, making appointments.
• Distributes sample medications to patients under the direction of the provider, explaining risks, benefits, side effects, and instructions.
• Follows up on ordered tests by obtaining results and relaying to the provider.
• Schedules appointments and ancillary services for related and follow-up patient care
• Collects and records patient history from visits with both internal and external offices and prepares chart prep for next office visit. This is to include patient allergies, medication list, patient, and family medical/surgical history.
• Collects, records, and maintains patient information and vitals for current office visit
• Conveys information to provider; obtains and reviews orders; seeks provider input as needed.
• Working under the general direction and instruction from the providers and RN on staff. Ability to work with minimal supervision.
• Assists providers with exams/procedures, including cleaning, preparing, and stocking exam rooms and equipment throughout the day.
• Places orders for clinical supplies with manager approval. Restock shelves and performs inventories. Assures sufficient and appropriate stock of supplies.
• Contacts insurance companies as needed for prior approval for procedures, tests, immunizations, etc.
• Properly collects and delivers specimens to hospital laboratory for testing.
• Manages stock of drug samples, assuming accountability for documentation, records, and inventory.
• Performs procedures such as injections, CLIA waived testing, and venipuncture.
• Follows proper protocol for all medical tests, procedures, and use of equipment.
• Protects patient rights to privacy and confidentiality.
• Attends Monthly/Quarterly Staff meetings
• Will perform Quality Control Report for Clinic Providers
• Performs related duties as assigned by Clinic Manager.
Educational Requirements and Certifications
• Graduate of an approved program of practical nursing.
• Current West Virginia Sate Nursing license is required.
• Must possess CPR certification.
• BLS Certification within orientation period.
Experience
• One-year experience in a clinic, emergency dept., or medical office setting, preferred.
• Phlebotomy experience required.
• Authorizations and referral experience is required.
• Patient scheduling knowledge is preferred.
• Demonstrated knowledge of general theory and practice
• Ability to work and deal effectively with others.
• Must be able to work with minimal supervision and adapt to each provider.
• Experience using EHR
• Demonstrated experience in developing and maintaining interpersonal relationship with co-workers
• Ability to remain calm in stressful situations.
• Must be available to work Monday through Friday.
