What are the responsibilities and job description for the Diabetes Educators Needed! position at Monitor My Health, Inc.?

Monitor My Health is a 501(c)3 non-profit with a mission to provide evidence-based health promotion programs for chronic disease prevention and management in the community and addressing the social determinants of health. Monitor My Health is looking for Diabetes Educators and Registered dietitians to provide Diabetes Self-Management and Education Support Services in the community (DSMES).

Key Responsibilities:

  • Education Programs: Deliver DSMES programs following evidence-based curriculum to individuals with diabetes, covering topics such as self-monitoring, medication management, nutrition, exercise, and emotional well-being.
  • Assessment: Conduct assessments to understand patients' educational needs, health status, and readiness to learn. Tailor education plans are based on individual requirements.
  • Individual Counseling: Provide one-on-one counseling to patients, offering personalized guidance on self-management strategies and lifestyle changes.
  • Group Sessions: Facilitate group education consisting of 9 sessions to create a supportive environment where individuals with diabetes can share experiences and learn from one another. Group sessions will consist of 10-15 participants and each cohort will meet weekly for 10 weeks.
  • Monitoring and Evaluation: Track patients' progress and outcomes, adjusting educational plans as needed. Evaluate the effectiveness of educational programs.

Experience: Previous experience in diabetes education or a related field is beneficial. Experience working with diverse populations and cultural competence is also desirable.

Qualifications:

  • Bachelor's or Master's degree in a relevant field (e.g., nursing, dietetics, health education).
  • Certification as a Diabetes Educator (CDE) is preferred.
  • Knowledge of diabetes management principles, including medication, nutrition, physical activity, and psychosocial aspects.
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively in a healthcare team.

Time Commitment: Once per week for about 1-2 hours for one class for 10 weeks. You can take as many classes as you can handle.

Location: In the community (senior centers, houses of worship, living apartments) in New Haven County.

Job Type: Contract

Pay: From $50.00 per hour

Expected hours: 1 – 10 per week

Work Location: In person
