What are the responsibilities and job description for the LPN position at Morningside House Senior Living?
Currently looking for an LPN to join our growing team!
Assist with administration of medications as ordered by a physician in accordance with any state and federal regulations and consistent with community policy.
- Assist with resident care as needed.
- Responsible for following up on residents who are on MLOA in hospitals or Rehab facilities.
- Responsible for leading change of shift report between outgoing shift and incoming shift.
- Respond quickly to all emergencies and provide oversight and coordination during emergency situations
- Monitors the emergency response system in the absence of the DHW or ADHW.
- Address any conflicts that arise from staff, residents, or families in the absence of the DHW or ADHW.
- Responsible for the admissions of new residents to include all admission assessments and documentation.
- Responsible for completing resident’s plan of care for caregivers.
- Responsible for clinical system tracking (i.e. wounds, infection control) under the direction of the DHW.
- Ensuring the setting up of charts for new residents and entering appropriate information into the EMAR
- Completing weekly care notes as well as weekly cart, Medication Administration Record, and chart audits and pulling expired and/or discontinued medications from med carts
- Ensuring all incident reports are completed accurately, responsible for faxing incident reports to QA Reader and completing investigations and follow-up
- Responsible for approving new medication orders and/or changes to medication and treatment orders on the EMAR
- Ensures that physician orders are transcribed to Medication Administration Record (MAR)/Treatment Administration Record (TAR) accurately
- Faxes all medication orders to pharmacy
- Ensures that all medication is available for administration to residents
- Reviews MARS/TARs and narcotic count weekly to ensure accuracy and completes medication audits using approved form per P&P
- Supervises assignment, tracking and ongoing maintenance for all department equipment to include walkie-talkies, pagers, and resident room keys
- Provide general consultation regarding health‐related concerns within the community.
- Record relevant information in the 24-hour Communication Log at the end of each shift.
- Record necessary information in resident chart related to change of condition or health updates
- Ability to communicate effectively and efficiently when listening, speaking, reading and writing.
- Notify physicians and/or family of any change in resident’s health and provides proper documentation.
- Review MARs/TARs daily to ensure compliance with medication administration and accurate documentation.
- Work as part of a team and handle multiple tasks safely and effectively while maintaining close communication with all departments and staff members.
- Provide educational meetings for residents and staff and attend all required training, in‐service and staff meetings.
- Responsible for providing oversight, supervision of and direction for care staff.
- Understand role in the Safety & Disaster Plan.
- Maintain confidentiality of residents’ and prospective residents’ information.
- Strive to maintain a safe working environment through the prevention of accidents, the preservation of equipment and the achievement of safe working practices.
- Maintain a positive and professional demeanor toward residents, visitors, families and co‐workers.
- Adhere to all policies and procedures of Morningside Elite Management.
- Perform other duties as assigned.
Qualifications/Skills/Educational Requirements:
- Maintains current state license as a Licensed Practical Nurse per state regulations.
- Must be Cardiac Pulmonary Resuscitation (CPR) certified
- License must be in good standing.
- This position requires a certain level of flexibility with work hours.
