Description

Under the direction of the Morongo Tribal Social Services (MTSS) Executive Director, plan, develop, organize, and coordinate the initiatives, activities, and operations related to the care and well-being of Tribal Elders. Identify and meet Elders' needs in partnership with Morongo Tribal Social Services and other resources in and out of the Reservation.

Act as an advocate and guide for Elders, assisting them in attaining independence, functional mobility, and self-determination, while addressing safety, security, health, and related wellness and life issues.

Develop collaborative relationships with Elders. Assist Elders in visioning for their care and future. Meet with and assess clients' needs and develop service plans, utilizing appropriate mental health, social, legal and health care services within the Department and the Morongo Tribe guidelines. Conduct regular case reviews and overall case management.

Determine when to recommend supportive counseling and / or education to assist clients and their families in recognizing and correcting personal, family, financial and social conditions contributing to problems; instruct clients on beneficial psychosocial and / or health care management; make appropriate referrals and monitor progress.

Assess need for in-home care, conduct home studies to ensure clients safety and wellbeing, conduct home visitations and companionship. Assist in crisis interventions including voluntary and involuntary hospitalization and placements as needed. Facilitate collaboration between health care providers, client and family. Help determine types of services, including insurance, home health, hospice and conservatorship.

Coordinate activities and partnerships between MTSS, legal department, tribal court, fire department, Morongo Tribal Police Department, Elder's program, and other Morongo and outside resources.

Engage, coordinate and monitor services including communication with outside providers, site visits, transportation, appointments, completing forms, assessing and securing placements and services. Maintain case files and contact notes. Update case status and provide reports as requested.

Requirements

  • Various areas in Aging Life Care : Health and disability; financial services, including federal, state, and tribal entitlements; legal; end-of-life care; family dynamics in aging life, crisis intervention, advocacy and local resources.
  • Various components / terminology of local, state, and federal social service programs, and appropriate forms used for each case and / or service provided
  • Awareness of impact of trauma on personal, family, and social functioning
  • Community resources providing social services and protection for Native American families.
  • Safety issues pertaining to home visits; potential for and acknowledgment of dangerous situations
  • Interpersonal skills using tact, patience, compassion, and courtesy
  • Effective oral and written communication skills
  • First Aid / Adult AED-CPR certified.
  • Valid CA Driver's License and insurable on company policy.
  • Any combination equivalent to a bachelor's degree from an accredited university in Human Services, Social Work, Psychology, or related field and five or more years of increasingly responsible experience in social services, case management, advocacy, etc.
  • Plus, minimum 3 years of experience with Tribes or supporting Native Americans.
  • Plus, experience in Eldercare and life stages / death and dying.
