Independent Contractor: Adult Case Management
Morrison Center is looking for Independent Contractors to provide Adult Case Management services in the Westbrook, ME area, in response to our expanding program. This role involves a mix of office work and traveling for home visits.
Scope of Responsibilities:
- Assist clients in achieving their goals by completing necessary tasks and requirements.
- Maintain documentation in compliance with regulatory agencies, such as the Department of Health and Human Services.
- Conduct monthly home visits.
- Develop and manage Person-Centered Plans (PCPs), assessments, and goals.
- Maintain effective communication and perform research as needed.
Qualifications:
- Minimum of 1 year of experience working with children or adults.
- Bachelor’s degree in any field.
- Experience with families and familiarity with PCPs and the Evergreen system is preferred.
Contract Details:
- Duration: This is a rolling contract that will be reviewed annually.
- Notice Period: Thirty days are required for termination of the contract.
- Hours: Flexible.
