We are seeking home providers in Arizona to share their homes with individuals with disabilities.
Become a Host Home/Shared Living provider with Mosaic at Home. You will maintain a safe and healthy home environment, assist with daily tasks, and foster independence. Our shared living service has improved quality of life for people with disabilities for nearly 40 years.
As a home provider, you will match your lifestyle with that of an individual with a disability, participating in activities together at home and in the community. Your duties will vary depending on the support level needed by the individual, from minimal to full support.
Responsibilities include:
- Providing support for daily tasks such as cooking, cleaning, and budgeting
- Assisting with medication administration, medical appointments, and overall health and well-being
- Documenting daily activities via web-based case management application
- Providing opportunities for community inclusion, integration, and socialization
- Communicating effectively with team members and supporting physical needs of the individual served
Additional responsibilities may include:
- Assistance with bathing, toileting, feeding, and behavioral support
- Providing support for medical protocols and caring for individuals with complex physical or medical needs
Requirements include:
- Residency in the state of Arizona
- Transportation and valid driver's license
- Home insurance and ability to pass State/Federal background check
Salary : $42,500
