What are the responsibilities and job description for the Licensed Practical Nurse (LPN) position at Mountain Community Health Partnership?

Mountain Community Health Partnership is seeking a full time Licensed Practical Nurse (LPN). The LPN Scope of Practice in all steps of the nursing process is limited and focused because by law, it is a dependent and directed scope of practice. LPN practice requires assignment or delegation by and performance under the supervision, orders, or directions of a registered nurse (RN), physician, or other person authorized by State law to provide the supervision. The LPN may be asked to work at any MCHP facility based on clinic needs. 


The LPN is responsible for utilizing the nursing process to provide nursing care to patients of MCHP under the direction/supervision of RNs, physicians, nurse practitioners (NPs), and physician assistants (PAs) in accordance with the NC Nursing Practice Act.  The LPN is a member of a care team lead by the healthcare provider and serves as a liaison between the patient and the healthcare provider for establishing and maintaining quality patient care.


LPN responsibilities include but are not limited to data collection of patients chief complaint, allergies, mediations, history, surgeries, immunizations, and vital signs; Updating medical information in the patient chart in the EMR at each appointment, checking alerts and reminders to ensure all recommended elements of care are offered and performed; Provide ongoing assessment of patient status for changes in condition; Participates in identifying patient/family needs and developing plans of care in collaboration with RN or Provider; Facilitates coordination of care by obtaining and tracking test results and consult records and communicating with healthcare providers, community organizations, health plans, facilities and specialists. 


Must be a graduate from an accredited school of nursing and possess a valid license as an LPN in the state of North Carolina, required. One year experience in medical/surgical nursing or clinical setting preferred.
