Integrated Clinical Services provides quality health services for people who experience barriers to accessing care. The vision of Integrated Clinical Services is to provide services that are integrated and connected to support optimal health, reducing wait times, and optimizing access.
HIV Health Services Center (HHSC) is a part of Primary Care. HHSC serves persons living with HIV. HHSC provides ambulatory outpatient medical care and supportive services including mental health and substance abuse counseling, medical case management, patient navigation, health education, HIV prevention and risk reduction, harm reduction and treatment adherence counseling. Our comprehensive, multidisciplinary model is tailored to meet our patients' complex health needs. This position is responsible for developing and delivering activities designed to promote individual and community health, specifically for people newly diagnosed with HIV and new to care at HHSC as well as people who have been out of HIV care for over a year and are re-engaging in care.
As the HIV Rapid Patient Navigator, you will:
  • Works with people newly diagnosed with HIV, new to the area or re-engaging in care from a variety of communities to assist them in addressing barriers to engagement in care and adherence to HIV medications.
  • Utilizes thorough knowledge of the needs and values of communities impacted by HIV and the Ryan White care continuum in order to follow grant required practices.
  • Uses targeted interventions to resolve complex problems with general guidance. Responds to direct referrals for HIV Rapid Start Navigation services from Intake Coordinator and Program Supervisor.
  • Provides intensive navigation services for new HHSC Rapid Start clients who have never taken HIV medications or been out of care for over a year including clients experiencing significant barriers to engagement in care and viral suppression, including homelessness, mental health issues, active substance use, and complex physical health issues.
  • Conducts outreach and provides navigation services to clients living with HIV who are eligible for the Rapid Start program. Coordinates and collaborates with community partners in referring clients to culturally specific resources
  • Responds to the needs presented by an individual client including those that arise from the client's cultural background and experiences, towards the goal of ending disparities and eliminating health inequities within this community.
  • Cultivates relationships with current and new community partners to support Rapid Start activities, including strengthening referral relationships. Performs community needs assessment and develops outreach plans specific to needs of newly diagnosed and new to care patients.
  • Coordinates and collaborates with community partners to inform them of HHSC services and develop rapid referral relationships.
  • Participates in HIV Clinic quality improvement activities.
  • Takes the lead on assigned rapid improvement cycles and projects. Utilizes relevant data in decision-making and planning.
  • Develops and maintains effective working relationships with team members and assigned communities.
  • Participates in regular team, group and individual case conferences. Maintains effective relationships and regularly collaborates and communicates with patients, community partners, and co-workers.
  • Communicates clearly and concisely, both orally and in writing. Based on own life experience, shares community perspectives and needs with colleagues in the health and social service systems. .
Minimum Qualifications:
  • Education/Training:Equivalent to completion of the twelfth grade AND
  • Experience: Two years of experience in community outreach services providing health information, advocacy, social support and assistance in using the health care system to groups and families; AND experience working with the community being served.
Other: BLS/CPR Certification (American Red Cross or similar organization)
Preferred Qualifications/Transferable Skills: You do not need to have the following preferred qualifications/transferable skills to qualify. However keep in mind we may consider some or all of the following when identifying the most qualified candidates. Please clearly explain on your application how you meet any of the following preferred qualifications/transferable skills.
  • Bachelor's Degree in Public Health, Social Work, or counseling
  • State of Oregon Community Health Worker (CHW) Certification (up to 1 year after hire to complete)
